Policies Regarding Your Care

scheduling an appointment:

In order to receive physical therapy treatment, all patients MUST have a referral from their physician.  A written order from your physician that is faxed or brought with you in person is acceptable.  Lifestyle Therapeutix also has a referral form that you are welcome to use.  You can pick that form up yourself, we can email it to you or fax to your physician.  Once that is received, an appointment can be scheduled to begin care.

cash and personal financial policy:

You are financially responsible for all services rendered to you in this office. Payment is made in full (100%) for each visit at the time of visit unless special arrangements have been made with your provider. While we are happy to bill your insurer, YOU are responsible for the cost of your care at Lifestyle Therapeutix.

CHECK-IN:

New patients should expect to arrive a little earlier for their first appointment in order to complete necessary paperwork.  Be prepared to provide your insurance card(s) and picture ID.

health education and wellness coaching:

ALL rehab patients receive health education and wellness coaching as part of their care.  If you are not a rehab patient and would like to schedule a Health Education/Wellness Consultation, please contact our office for details.

cancellations:

No charge will be made for cancellations or appointment changes if 24 hours notice is given. For cancellations with less than 24 hours notice full price will be charged and YOU are responsible for paying it. Payments need to be made in full by you at the time of service for service to continue.
